1. Completed application form;
2. 2 passport size photos (1 certified by a Justice of the Peace - JP);
3. 2 Recommendations - may be from a Justice of the Peace, Lawyer, Medical Doctor, Principal, Gazetted Rank Police Officer (not below the rank of Deputy Superintendent), Member of the J.D.F. (Major and above);
4. Proof of age (Passport or Birth Certificate. Original & Photocopy);
5. Original & Photocopy of both licence fee receipt & finger print receipt;
6. Proof of income;
