6. Exportation of Ammunition from Belize

3) Each export attachment shall contain the following minimum information:-
a) Shipment information - the lot numbers of ammunition and quantity being shipped, with each classification - description (as per bill of laing if required by the exporting and/or importing countries), date of shipment, exit port, routes planned, and all specific shipping modes and shippers;

b) Shipper's information - for each shipper identified in paragraph (a) above, the name, postal and/or physical address, telephone and facsimile address of the shipper or the sipper's representative if the shipper is a commercial or government operator;

c) information on prior shipments, if any, including dates of exit of previous shipments, quantities of ammunition shipped, by classification - description per shipment, cumulative quantities of all shipments shipped prior to the applicable shipment, and the name of the shipper.
